On May 2, reports indicated continued and intensified military operations in the No Fire Zone areas during the final stages of the conflict in northern Sri Lanka. The last remaining hospital in Mullivaikkal was struck twice by artillery fire in the morning, resulting in significant civilian casualties. According to humanitarian and monitoring sources, at least 64 civilians were killed and 87 others injured in the attacks on the makeshift medical facility located within the government-declared safe zone. The repeated shelling of medical infrastructure raised serious concerns regarding the protection of civilians and the adherence to international humanitarian norms.

In a separate development, tensions extended beyond the conflict zone. Activists belonging to the pro-Tamil nationalist Periyar Dravidar Kazhagam (PDK) in Tamil Nadu, India, reportedly attacked a convoy of Indian Army trucks allegedly transporting weapons to Sri Lanka. Five military trucks were damaged during the incident, and sources in Chennai claimed that mortar shells were found in the vehicles. The event reflected growing regional political unrest and opposition linked to the ongoing conflict.
